A copy is provided to you at your first appointment, is posted in the office, and is available on request at any time. Call (210) 824-3243 and we will send you one.
How we use your health information
We use and disclose your protected health information for treatment, payment, and health care operations — for example, to plan and provide your dental care, to bill your insurance, and to run the practice.
Other uses and disclosures generally require your written authorization, which you may revoke. A limited set of disclosures is permitted or required by law without your authorization, such as public health reporting or responding to a lawful court order.

- A paper copy of the Notice. You may request one at any time, even if you agreed to receive it electronically.
- Notification of a breach. We will notify you if a breach compromises the privacy or security of your information.

If you believe your privacy rights have been violated, you may raise it with us and you may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be retaliated against for filing a complaint.
